Job Summary:

Receives referrals and makes or schedules initial patient evaluation visits as needed.
Attends case conference meetings, and takes report on patient's progress with agency personnel to facilitate coordination of care.
Coordinates transfer and discharge of patient from agency services as indicated by the physician.
Conducts a minimum of one chart audit per month and reports these findings to the Director of Nurses, Branch Manager, or appropriate agency personnel.
Reviews and provides supervision for each patient's plan of care, through review of services, diagnoses, medications, procedures, progress, clinical course, and goals.
Assists the Director of Nurses and Branch Manager in planning, implementation and evaluation of in-service and continuing education programs for staff.
Schedules nursing and aide visits and makes necessary changes as needed. Schedules aide care supervision visits for each patient at least every 14 days.
Provides services within the scope of practice as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team.
Continually evaluates the patient’s nursing needs. Instructs and supervises the patient’s family in the care of the patient and maintenance of a healthy environment for the patient.
Observes, records, and reports to the supervising nurse and/or physician the patient's response to treatment and changes in the patient's condition.
Participates in staff conferences regarding patient care.

The Occupational Therapist will test and evaluate patients' physical and mental abilities and analyze the data to determine realistic rehabilitation goals.  You will select activities necessary to help individuals learn life-management skills within limits of their mental and physical capabilities.

Notifies the Reimbursement Manager if there are overpayments and/or duplicate payments for the same service. Transfers payments belonging to other OCE offices within 48 hours of receipt. Follow up on invoices submitted to ensure prompt and timely payment. Calls to verify that claims submitted were received and are in processing. Sends letters to the patient or responsible party when their insurance carrier fails to make payment reasonable time frame. Generates and mails statements and collections letters per Walgreens-Option Care policy. Follows-up on all denials within 48 hours of receipt. Performs all appeals as necessary. Maintain confidentiality of patient and proprietary information. Observe legal and ethical guidelines for safeguarding the confidentiality of patient and proprietary Walgreens-Option Care information.

POSITION SUMMARY:
We are seeking an RN to provide direct nursing care to residents as a Traveling RCM in skilled nursing facilities in Metro Portland, Oregon and/or Washington.
We will work with your schedule and you can work as many or as few shifts as you prefer.

-Essential Nursing Functions-

RCM must evaluate and assess residents' needs, including physical and mental health, family and community resources, physical environment and finances. A plan of care for residents will be developed by RCM based on a close assessment of residents requirements. Arrangements need to be made to implement that plan. Once these arrangements are in place, the RCM can coordinate all services to insure the resident's health, safety and general well being. Often, there needs to be a continuous monitoring and periodic reevaluation to make changes as needed.
